PhpNuke UploadV2 Script With a Security Fix

As most of you know I have written a very simple Upload module and I made a
security mistake by giving out instructions that could compromise a folder on
the module. So I have included a new instructions, added a new feature to limit
the file size to be uploaded and also made it more simpler to edit as well as
allowing you to customize your own error messages. I am welcome to new
ideals to make this web script more robust.
